Most of our agreements are universal (for all campuses and across all health disciplines) and evergreen (no expiration date, but may be terminated by either party at any time).
To determine whether we have an agreement in place with the healthcare agency, look at the expiration date in the search results.
- Evergreen - An agreement is in place. It is okay to send students to the agency.
- In Process - The agency has not returned the agreements and/or provided an insurance certificate. It is not okay to send students to the agency. Please call 480-731-8881 to inquire about the status.
- Canceled - Agency terminated agreement or unable to finalize.
You may search for a single clinical agreement or all clinical agreements that apply to a specific health discipline and/or campus. At the "Site to Search" pop-up menu, select "All Sites" unless there is a specific agreement with your campus only. In the "Keywords" field, type in the name of the agency, the health discipline, or zip code (do not use any punctuation marks, including periods, commas, etc.). Click the "search" button. Appropriate clinical agreements will display.
